Andrew Paul Slavik, 46, of Olathe, passed away on April 22, 2018 in Olathe, KS.
He was born on October 7, 1971 to Donald and Juliana (Moravec) Slavik (Alexandria, MN) in St. Paul, MN.
Andrew graduated from Jefferson High School in Alexandria, MN in 1990, and received his Doctorate degree from Logan College of Chiropractic in 1996.
In 2014, Andrew joined Cleveland University Kansas City, and most recently became Assistant Dean of Clinical Education. Prior to that, he owned and operated his own private practice, Slavik Family Chiropractic from 1996-2014.
On October 24, 1992, he married Kristin Ann (Peterson) and to this union two children were born, Katelyn and Mitchell.
Andrew is survived by his loving wife, Kristin, his children Katelyn and Mitchell Slavik, his parents, Donald Slavik and Juliana Slavik, siblings Matthew (Cathy) Slavik, Steven (Sandra) Slavik, Robert (Stephanie) Slavik, and Jennifer Slavik, and 14 nieces and nephews.
He was preceded in death by a niece, Hannah.
A memorial service will be held on Friday, April 27, 2018, 3:00 pm at the Penwell-Gabel - Olathe Chapel, Olathe, Kansas, with visitation an hour prior to service time.
Memorials are suggested to the Dr. Andrew Slavik Scholarship Fund at Cleveland University Kansas City.
